Intensive Case Manager

Based out of the Cornwall Office
35 hours per week
Contract from July 26, 2010 to June 17, 2011

CMHA is a community-based agency dedicated to providing community support services to persons living with a mental illness and/or concurrent disorder. We are currently recruiting an Intensive Case Manager with demonstrated work experience in mental health and addictions to join our Cornwall team.

Responsibilities:
To provide community support services and intensive case management with the goal of independent living and community integration.

Requirements:
• University degree in the Social Sciences/Health field with two years experience as a Case Manager working with people with emotional/psychiatric difficulties or equivalent combination of education and experience may be considered
• Advanced language skills in both French and English are required.
• Valid First Aid certificate
• Valid driver’s license and access to a vehicle

Salary scaled, subject to union contract, commences at $43,898.

Intensive Case Manager (2 positions based out of Cornwall)
1 Permanent Full Time
1 Contract 21 hours per week – September 2010 to March 31, 2011

We are currently recruiting two Intensive Case Managers with demonstrated work experience in mental health and addictions to join our Cornwall team.

Responsibilities:
To provide community support services and intensive case management with the goal of independent living and community integration.

Requirements:
• University degree in the Social Science/Health field and two years experience as a Case Manager working with people with emotional/psychiatric difficulties, or equivalent combination of education and experience may be considered.
• Demonstrated abilities in working with minimum supervision, working alone and as part of a team.
• Demonstrated organizational skills.
• Possess a valid Driver’s License and access to a vehicle.
• Must have valid First Aid certificate.
• Advanced language skills in both French and English are required.
• Ability to work flexible hours (evening and weekend hours required).

Salary scaled, subject to union contract, commences at $43,898.
